Inflatable vs malleable penile implant

Two device classes, two quite different day-to-day experiences. The choice is less about quality than about fit.

The first decision in implant surgery is not the brand — it is the class of device. Inflatable and malleable implants solve the same problem in very different ways.

How each one works

A three-piece inflatable implant has cylinders in the penis, a pump in the scrotum and a reservoir of saline. You squeeze the pump to create an erection and press a release to deflate it. A malleable implant is two bendable rods: the penis stays firm at all times, and you bend it up for intercourse and down afterwards.

What the inflatable device gives you

  • A soft, natural-looking flaccid state with nothing to conceal.
  • Girth expansion when inflated, and rigidity you control.
  • The closest approximation to a natural erection currently available.

What it asks of you

  • Enough hand strength and dexterity to operate a scrotal pump.
  • Acceptance that it is a mechanical device that can fail and may need revision.
  • A longer operation with more implanted components, and space for a reservoir.

What the malleable device gives you

  • Simplicity. Nothing to inflate, nothing to activate, no pump or reservoir to fail.
  • The easiest device to use, which matters with arthritis, limited dexterity or cognitive difficulty.
  • A shorter, less complex operation.

What it asks of you

  • The penis remains firm at all times, so clothing needs more thought.
  • No girth change, and an erect state less like a natural erection.
  • Long-term attention to the tissue under constant pressure.

How the decision is actually made

Hand function is often the deciding factor, followed by anatomy — significant scarring or difficult reservoir placement can favour a malleable device. Beyond that it comes down to what you value: the natural flaccid state and controllable rigidity of an inflatable device, or the simplicity and mechanical reliability of a malleable one. Both are legitimate choices, and a surgeon who offers only one of them is not really offering a choice.
